The purpose of this study was to identify the prevalence of major chronic diseases affecting a group of Maroons known as "Mola", located in northeast Pará, relating them to the Social Determinants of Health (SDH). We observed that there is high rate of chronic diseases among these rural black populations in comparison to the rest of Brazilian population. 87.08% of adults has chronic problems related to the digestive system, 50.9% had skin diseases, 48.08% presented some level of pre-hypertension or hypertension and 43.63% have eye disorders. We also analyzed aspects related to comorbidities such as smoking, alcoholism, obesity and socio-ecological aspects, which point to the existence of a strong relationship between the prevalence of chronic diseases and the SDH.
